Generally bodily injury by accident must occur during the policy period. Bodily injury by disease must be caused or agitated by the conditions of employment with the employees last exposure being during the policy period.
Coverage and benefits paid are state mandated, and the exact qualifications for when Workers Compensation is required can change based on location state, company type (e.g. a sole proprietor vs an LLC), etc. Coverage may apply to medical treatment, loss wages, and rehabilitation among other things.
